Community Experience Program
PROGRAM MISSION:
Kuhn’s Meriden based Community Experience Program strives to provide individuals with disabilities the opportunity to learn appropriate social skills and behaviors through a variety of community based recreational activities. Participation in the program allows individuals to address specific programmatic goals through preferred activities within Kuhn and the Greater Meriden community with the support of enhanced staffing and guided social activities.
PROGRAM ELIGIBILITY:
- Verified Funding Source
- Minimum age 18 years
- Impairment in physical, cognitive, social, or behavioral functioning
- Able to benefit from services
ADMISSION INFORMATION REQUIRED:
- Document verifying disability and level of functioning
- Most recent physical exam, preferably within last year
- Recent psychological/social information, preferably within last 2 years
- Information regarding previous day services & supports
- All academic/educational history when applicable
- All appropriate agency forms completed, signed, and dated by participant or legal guardian and funding source, if applicable, prior to program entry

- Individualized Service Planning
- Behavior Management
