Supported Employment (Middletown)

To assist individuals with severe and prolonged mental illness, through the provision of individualized supports, to obtain and maintain competitive community employment so as to achieve their maximum level of independence.

ELIGIBILITY REQUIREMENTS:

• Eighteen years or older and a verfied funding source

• Resident of Middlesex County, Lyme or Old Lyme

• Meets the DMHAS definition of severe and prolonged mental illness as well as, definition of poor

• Receiving services from a DMHAS funded clinical / case management program

• Impairment in vocational functioning

• Desire to become employed

Application for Supported Employment services are accepted from mental health service providers and should be submitted to:

Self-referrals will be considered for admission in those cases where documentation can be obtained that substantiates the individual’s eligibility for the program.

Requests for additional information will include a recent psychosocial summary/assessment medical examination report and, any existing vocational information. The Kuhn Program Manager is responsible for obtaining DMHAS eligibility approval through River Valley Services prior to admitting an applicant to the program. All applicants will be interviewed to assess their interest in competitive employment.
