STATUS:              Exempt

STARTING SALARY RANGE: $23,500-$28,425/yr.

 

SUMMARY:

Responsible for the training and orienting of direct care staff as well as providing on-going training and skill enhancement for all direct care staff.  Responsible for assisting direct care staff with the development of Individual Program Plans.  Prepares reports for and participates in team meetings for case load.  Works in conjunction with supervisors to meet day to day scheduling needs.

SUPERVISORY RESPONSIBILITY:           None

ESSENTIAL DUTIES/FUNCTIONS:

This position requires such typical work operations and responsibilities as, but not limited to:

1. With input from stakeholders, develop Participant Program Plans including goals and objectives. Develop Participant Behavior Plans as appropriate.  Coordinate and attend all participant related meetings.

2.  Review and edit participant programs to ensure each program is in compliance with “best practices”/ industry standards (i.e.: participant input is included, objectives are measurable and specific to the individual).  Oversee implementation of program plans and monitor due dates of all participant program reviews to ensure programs are completed in a timely manner.

3. Maintain all participant Master Files and related records such as, DDS Emergency Fact Sheets and participant vacation schedules, in an organized and current manner.  

4. Work as a liaison between Kuhn and stakeholders including but not limited to family members, DDS staff and, residential providers to ensure that participants’ needs are met.

5. Provide agency orientation to new program participants and ensure completion of all necessary Intake paperwork.

6. Work in collaboration with other Kuhn staff to insure compliance with Department of Labor standards.

7. Work as a substitute direct care staff at the request of supervisor and/ or when dictated by staff coverage needs to include job coaching and driving duties.

8. Assist with on-going training and skill enhancement activities for direct care staff including educating staff with                respect to programmatic issues and agency policies.   

 

REQUIREMENTS:

1. Strong verbal, written and organizational skills, behavior management experience and the ability to provide customer service.

2. Bachelors Degree in Rehabilitation or human services related field OR Associates Degree plus minimum of 2 years of human services/ vocational rehabilitation work experience (including writing behavior and individual service plans) required.

3. Obtain and/or maintain CPR Certification, First Aid Training and, PMT training.

4. Possess basic computer skills.

4. Valid Connecticut drivers license (with Public Passenger Permit endorsement).

5. Available means of transportation.

6. Automobile insurance in force per Kuhn's specifications when using personal vehicle for work related purposes.

PHYSICAL REQUIREMENTS:

1. Ability to drive Kuhn Employment Opportunities, Inc. vehicles.

2. Ability to perform physical requirements at supervised work sites.
