SUMMARY:

The Vocational Specialist is responsible for assisting individuals with severe and prolonged mental illness to become competitively employed in the community and to reach their maximum vocational potential.  As a team member of the Gilead Assertive Community Treatment (ACT) program, the Vocational Specialist will focus on specific areas related to supported employment for ACT clients including engagement, assessment, job development, job placement, job coaching and benefits counseling.  Must be able to provide employment related support to individuals with a wide range of skills/abilities.  Must be able to be flexible with work schedule to meet the needs of program participants and their employers.  Must be able to communicate effectively with other service providers to coordinate service delivery for program participants.

 

ESSENTIAL FUNCTIONS & DAILY TASKS:

This position requires such typical work operations and responsibilities as, but not limited to:

  • Establishing and maintaining contacts with business and industry; insuring job development; maintain written records of such contacts.
  • Providing counseling and support to identify/establish vocational goals and job interests of the participant, as well as, administering/arranging for interest/skill assessments as needed, at all levels of Stages of Change in relation to employment. Vocational work with client may often be done less directly, as client needs evidence.
  • Obtaining employment opportunities for program participants via job development activities and maintaining effective communication with employers.
  • Providing or preparing for on-the-job support, job seeking skills and assistance with transportation arrangements necessary for the participant to be employed.
  • Providing or preparing for on-the job training; assisting with adjustment and transition to the work environment; supporting and/or educating employers or coworkers with respect to the participant’s strengths and needs.
  • Providing transportation for program participants during the job development process or in other circumstances when public transportation cannot be arranged.
  • Providing or facilitating ongoing supports and promoting independence; assisting participants, in conjunction with other service providers, to maintain a balance in his/her life with regard to work and other issues.
  • Representing Kuhn at seminars/conferences such as trade and human services sponsored training events, staying abreast of latest human services approaches/techniques and labor market trends.
  • Advocating and communicating with other agencies with regard to confidentiality and participant needs.
  • Provide expert consultation to the ACT staff through modeling, consultation, cross-training and participation in all team meetings and treatment planning meetings for clients with vocational goals.
  • Function as part of the treatment team, participating in case conferencing, daily treatment team meetings, ACT clinical and other treatment related meetings.  
  • Carries an individual caseload of ACT clients.
  • Demonstrates proficiency and understanding of the Fidelity Model as it relates to the Program.
  • Responds to after hours calls for evenings, weekends and holidays, as needed, on a rotating on-call basis.
  • Core services of the Vocational Specialist will include: engagement, vocational assessment of the client’s abilities, job development of the client, job placement (including classes and return to school), job coaching and follow up supports (including supports in academic settings), and benefits counseling.
  • Makes individual program percentage of required face to face or phone contacts per week and works in coordination with the agency Administrative Support Staff for proficient service delivery.
  • Demonstrates proficiency and understanding of the ACT Fidelity Model as it relates to the Program.
  • Provides competent Recovery oriented clinical case-management services by the demonstration of: intervention and support, inclusion of family/friends/community, dignity and care, supportive counseling and medication self-administration where applicable.
  • Carries out all phases of clinical services, including intake, engagement, assessment, treatment planning, continued services, transitioning and discharge processes. Facilitates step down to less intensive support services when necessary.
  • Consults and monitors medical and mental health needs regarding risk management. Implements risk management plans as necessary.
  • Attends all mandatory KUHN staff meetings and all-agency events, with co-supervision of KUHN employee by KUHN supervisor and Gilead ACT supervisors at least quarterly.
  •  Other duties as assigned.

 

BASELINE SKILLS & JOB REQUIREMENTS:

  • Bachelor’s Degree in Rehabilitation, Human Services or related field OR combination of a 4 year degree in a non-related field with related work experience.  Strong verbal and written communication skills necessary.  Some computer experience.
  • Minimum of one year experience providing employment services.
  • CPR certification and First Aid training required.
  • Valid Connecticut driver’s license.
  • Available means of transportation.
  • Automobile insurance in force per Kuhn’s specifications when using personal vehicle for work related purposes.
