SUMMARY:

Responsible for the training of appropriate social skills and behaviors for individuals with disabilities including instruction and supervision.  Facilitator is “hands on” person and will be required to perform all aspects of behavioral programming & follow through with teaching strategies, as well as, attend to personal needs of persons served as required.

SUPERVISORY RESPONSIBILITY: None

ESSENTIAL DUTIES/FUNCTIONS:

Position requires such typical work operations and responsibilities as, but not limited to:

1.         Plan and implement daily programmatic activities, such as but not limited to; daily set      up, recreational, ADL, OT/PT guidelines, and behavior programs

2.         Provide input to development of client program goals and behavior plans with Program Manager or designee.        

3.         Responsible for timely, accurate record keeping functions required by Kuhn and each       client’s program, such as but not limited to:  attendance, charting, activity logs,      program plan documentation, and communication logs.

4.         Responsible for transporting clients to and from Kuhn, community recreation, to include              driving, mobility training and other arrangements and documentation.  Also, responsible                for van mileage paperwork (recording mileage, gasoline receipts, passengers, etc.).

5.         Engage clients with meaningful, quality activities appropriate to individual interests and ability. Assist clients with personal needs as required such as, toileting, feeding and mobility.

6.         Active communication daily, or as designated, with Program Manager regarding behavior, health, daily activities, and any other necessary issues directly regarding clients.

OTHER DUTIES:

1.         Works with and assists others as required.  Performs additional duties at the request                of the management.

2.        Responsible for performing work duties in a safe manner, adhering to all established                 safety procedures and providing input into enhancing overall company safety.

3.         Attends job relevant training sessions.

REQUIREMENTS:

1.         Employs open, positive verbal and written communication skills.

2.         Ability to transport clients.

3.         Obtain and/or maintain PMT (Physical/Psychological Management Training)

4.         Available means of transportation.

5.         High School/GED Diploma plus a minimum of one year experience working with                        persons with developmental disabilities.

6.         Obtain and/or maintain CPR certification/First Aid Training.

7.         Valid Connecticut drivers license (with Public Passenger Permit endorsement).

8.         Automobile insurance in force per Kuhn’s specifications when using personal vehicle                 for work related purposes.

 

PHYSICAL REQUIREMENTS:

Able to physically transport, perform PMT, CPR and implementation of behavior plans which may require physical assists and emergency restraints.
