 SUMMARY:

Responsible for the Intake & Orientation process for new program participants and for maintaining participant related master files/ records in an organized and current fashion.  Acts as a liaison with Department of Developmental Services (DDS) staff, Residential staff, family members and other stakeholders.  Participates in team/ participant specific meetings and advocates on behalf of program participants to ensure that their vocational needs are met.  Assists with the training and orientation of direct care staff with respect to program participant-related paperwork, goals and behaviors, as well as, assist with on-going training and skill enhancement for direct care staff.  Responsible for the development of Participant Program Plans and participant-related reports with input from direct care staff.  Works in conjunction with supervisor to meet day-to-day scheduling needs and to complete various reports required by funding source(s).

ESSENTIAL DUTIES/FUNCTIONS:

This position requires such typical work operations and responsibilities as, but not limited to:

1.         With input from stakeholders, develop Participant Program Plans including goals and objectives. Develop Participant Behavior Plans as appropriate.  Coordinate and attend all participant related meetings.

2.         Review and edit Participant Programs to insure compliance with “best practices”/ industry standards and that all DDS funding requirements are met (i.e.: participant input is included, objectives are measurable and specific to the individual).  Oversee implementation of program plans and monitor due dates of all participant program reviews to insure programs are completed in a timely manner.

3.         Maintain all participant Master Files and related records such as, DDS Emergency Fact Sheets and participant vacation schedules, in an organized and current manner.  

4.         Work as a liaison between Kuhn and stakeholders including but not limited to family members, DDS staff and, residential providers to ensure that participant’ needs are met.

5.         Assist with DDS attendance and other required reports as assigned by supervisor.

6.         Provide agency orientation to new program participants and insure completion of all necessary Intake paperwork.

7.        Work in collaboration with other Kuhn staff to insure compliance with Department of Labor standards.

8.         Work as a substitute direct care staff at the request of supervisor and/ or when dictated by staff coverage needs. 

 OTHER DUTIES:

1.         Works with and assists others as required.  Performs additional duties at the request of the management.

2.         Responsible for performing work duties in a safe manner, adhering to all established safety procedures and providing input into enhancing overall company safety.

3.         Attends job relevant seminars/conferences.

REQUIREMENTS:

1.         Strong verbal, written and organizational skills, behavior management experience and the ability to provide customer service.

2.         Associates degree and/ or a minimum of 3 years of human services/ vocational rehabilitation work experience (including writing behavior and individual service plans),   Bachelor’s degree preferred.

3.        Working knowledge of Microsoft Word and basic computer skills.

4.         Obtain and/or maintain CPR certification, First Aid Training and, PMT training.

5.         Valid Connecticut drivers license (with Public Passenger Permit endorsement).

6.         Available means of transportation.

7.         Automobile insurance in force per Kuhn's specifications when using personal vehicle for work related purposes.

8.         Possesses basic computer skills.

 PHYSICAL REQUIREMENTS:

1.         Ability to drive Kuhn Employment Opportunities, Inc. vehicles.

2.         Ability to perform physical requirements at supervised work sites.
