Group and Independent Supported Employment Programs

Kuhn Meriden assists individuals with all levels of developmental disabilities to participate in employment through group and individual supported employment opportunities in community work enclaves and independent work sites. 

PROGRAM ELIGIBILITY:

• Verified Funding Source

• Minimum age 18 years old (16-17 year olds accepted when funded through a Board of Education)

• Impairment in vocational functioning

• Demonstrates interest desire to attain employment or demonstrates a willingness to work

• Able to benefit from services

ADMISSION INFORMATION REQUIRED:

• Documentation verifying disability and level of functioning

• Most recent physical exam preferably within the last year

• Recent psychological /social information preferably within the last two years

• Information regarding previous employment services & supports, as available

• All academic/educational history when applicable

• All appropriate agency forms completed, signed, and dated by participant or legal guardian and funding source prior to program entry

• “Intake interview” completed with referred individual by Kuhn designated staff and other team members when applicable.

• Job Coaching

• Job Support

Individualized Service Planning

• Behavior Management, as applicable
